The Hiring Incentives to Restore Employment (HIRE) Act Employee Affidavit needs to be completed by all qualified employees. Employees are considered qualified if they were not employeed for sixty (60) days or worked less than 40 hours in the sixty days prior to being employed by Greater Houston Pool Management. W-11 FORM
