Certificate Replacement Request Form

If you have lost your American Red Cross certificate(s), you can contact the American Red Cross office at 713-729-1620 to request a replacement certificate or get it through GHPMI. We can only replace American Red Cross certificates, not YMCA or Ellis and Associates.

It is recomended to start with the American Red Cross office if you took the class with someone other than GHPMI. ARC will also charge a fee for processing your request. If you want GHPMI to research and replace your card(s), please see the information below.

The administrative fee for GHPMI to replace your certificates is $10.00 per person. You can request to replace multiple cards for $10.00. Example: For a Lifeguard Training class you are issued at least 2 certificates. You would receive the Lifeguard Training/FA and CPR/AED for the Professional Rescuer certificates for $10.00.

If you have taken a class with another company and we are not able to track down the information to issue another card, then we will refund half or $5.00 of the replacement fee. It would be best to start with the American Red Cross if this is the case.

Steps to receive a replacement certificate from GHPMI:

1. Fill out and submit the form below.

2. You will receive an email with a payment request. Complete payment by CC on-line.

3. GHPMI will research and verify enrollment and completion of class. GHPMI will mail the American Red Cross replacement certificate(s) to you. You can also make arrangements to pick up the certificate(s) from our office in Cypress.

GHPMI will not start processing your request until payment is made.
